Medication Guidelines for School
Any medication needs to be in the original/prescription container with the proper label. Our district physican has ordered TYLENOL, ADVIL, BENADRYL and TUMS to be given with written parent consent. (See OTC medications) Any other medication needing to be administered in school requires a DOCTOR'S ORDER. See form below.

Consent for OTC medications
I have orders from our district physician to administer Tylenol, Advil, TUMS and Benadryl as needed for a student WITH WRITTEN PARENTAL CONSENT. See the attached form to circle what you give permission to be administered to your child if needed. YOU WILL BE CONTACTED BEFORE ADMINISTRATION OF NON-EMERGENT MEDICATIONS.
